How do you move on to a leadership position

My organization doesn't have a clinical ladder. So how do you build up your leadership skills and eventually get into management and supervisor positions? Do you just put in a number of years in your job and then apply to a management position?

How can a RN be specially trained to perform duties outside of scope of practice AND not be licensed (APRN), even if it's within the employer's protocol of duties? Isn't that risking one's license? I need some education on how this is possible.
