Say you have to chart a complete assessment q2h...busy at 4 pm, catching up on charting at 6 pm...so charting for both 4 pm and 6pm at the same time...but also "precharting" the 8 pm assessment with the intention of going back if anything changes. Couple questions...

Like looping IV tubing back into itself (against our policy, and they went to great lengths to hand out caps to carry in pockets). And not even cleaning off the port you looped it into!
Disconnecting a running IV to take a pt to the bathroom.. and just leaving the bare end hanging from the pole. Don't interrupt IVFs ! Take the blinking pump with you!

Last week the NNP and I attended the delivery of a baby with a known abnormality. The infant was to be DNR status. The delivery became very complicated and traumatic. It became an emergent situation and I honestly can say it was the most horrific thing I have ever witnessed. I am feeling quite traumatized and honestly pretty shook up.

Okay, we all know how easy it is start an IV, give an injection or perform some other type of procedure on a patient. However, what happens when WE are the patient? Are we the brave patient who grins and bears it. Or are we the pt that screams when you even discuss what is about to take place? What do you do?
